Posterior Plating in Distal Fibular Fracture

Abstract
Purpose: To report the technical experience of posterior plating for the distal fibular fracture.

Materials and methods: 20 W eber type-B fibular fractures w ere included in this study, w hich were treated with the posterior plating. 1/3 sem itubular plate was used and orientation of allscrew s were intended to be perpendicular to the plate as possible. Fixation stability and m aintenance of reduction after plating was assessed m anually in the operating field. Clinical results were evaluated at least 1 year after operation, using Am erican O rthopaedic Foot and Ankle Society (AFO AS) Ankle-H indfoot score.

Results: 5 cases were firm ly stabilized without using any lag screw or fixation of distal fragm ent. For im proving stability or achieving proper reduction, a lag screw was placed posteroanteriorly through the plate in 14 cases. Anteroposterior interfragm entary fixation in 1 case before plating,and contouring ofthe plate in 3 cases w ere needed in cases ofw hich the posterior plating im peded reduction of distal fibular fracture. In all cases, fracture was stabilized w ithout fixation through the m ost distal hole. There were no m ajor postoperative com plications. AFO AS score w as 95.5¡¾5.2.

Conclusion: The posterior plating technique for distalfibular fracture is regarded as a recom m endable option. Additionalfixation with interfragm entary screw or contouring ofthe plate,how ever,w ould be needed in som e cases to achieve anatom icalreduction or sufficient stability.
